本宮山地蔵院は 円城竹林96番地 北緯34度53分28秒・東経133度48分20秒に建つ天台宗の寺院で、連絡先の電話番号は 0867-34-0046です。
本尊は聖観音菩薩(です。本宮山円城寺の福蔵坊が 前身です。山門は 一間一戸の 鐘楼門です。門扉は いつも閉まっていますが、西の裏口の戸は 開いています。薬医門の横の潜り戸の門から 境内に入れます。境内には 聖観音菩薩立像 釈迦三尊と思われる坐像 多社宮が１社祀られています。Honguuzan-Dizou-in/temple which belong to Tendai-syuu/sect, is built on 96 Enjyou-Tikurin The principal image is Syou-kannon-bosatu “Aryavalokitesvara, the actual figure or shape of Kannoon-bosatu” The predecessor of Dizou-in/temple was Hukuzou-bou temple of Enjyou-ji/temple A standing statue of Syou-Kannon-bosatu "Kannon-bosatu not transformed", a seated statue of Shakamuni triad and a shrine consisting of many chambers, are enshrined in the precincts

加茂八十八ヶ所霊場巡礼８８番奥の院地蔵院札所巡礼で 導師によると 地蔵菩薩を 本尊とするとの事です。The principal image is Kṣitigarbha/Dizou-bosatu 本宮山円城寺：北緯３４度53分31秒に建ちます。行基菩薩が開いたとされ、かつては 本宮山にあり正法寺と呼ばれていましたが、鎌倉中期の大火により消失し 現在の場所に再建し 円城寺と名を改めました。最盛期には僧坊１４坊 末寺６ヶ寺を誇りましたが、現在は地蔵院と医王院を残すだけとなりました。
